I understand that telehealth involves the use of electronic communications for medical
evaluation and treatment. I consent to receive healthcare services via telehealth through CLT
Wellness Services LLC. I understand:

  1. I may decline telehealth at any time.

  2. My provider will maintain confidentiality per HIPAA.

  3. Telehealth may not replace in-person care in all cases.

  4. Prescriptions will be provided when medically appropriate.

  5. I must be physically located in a state where my provider is licensed.
